Reminder: Important Upcoming Vaccination Deadlines

This is a reminder to all students, staff, and faculty that under a mandate from The CSU Chancellor, all students, staff, and faculty must be either fully vaccinated or qualify for a medical or religious exemption. Humboldt has set the deadline of September 10.

Additionally, all employees  are also required to submit a vaccination self-certification, regardless of whether or not  they are accessing campus facilities. The certification form includes submitting vaccination information, a request for exemption, and/or the option to certify you won't be accessing campus facilities.

Students can call Student Health & Wellbeing Services at (707) 826-3146 to make vaccination appointments. All students, regardless of intent to access campus facilities, should submit their status or request an exemption through the Student Health Portal. Students who are not accessing campus facilities should indicate that they are not accessing any campus facilities on the exemption form.

  • Submit your vaccination information through your Student Health Portal.
    • Log in to myHumboldt
    • Go to: your Health Portal
    • Go to: Document Upload 
    • Choose Documentation Type: Covid Vaccination Card
    • Upload a picture or scan of your Vaccination Record Card
  • To request a medical or religious exemption or attest to your “off-campus / remote” status:
    • Log in to myHumboldt
    • Go to your Health Portal
    • Go to the Forms tab
    • Fill out the form "Exemption from COVID-19 Immunization Requirement (HousingAthletics)"
    • Contact the Student Health & Wellbeing Services with any questions: (707) 826-3146
    • You’ll be contacted by the Student Health Center if your request is rejected.
  • If you are granted an exemption, you will be given accommodations, which include required weekly testing


ALL employees must submit a self-certification. If you are not accessing campus facilities, please indicate that you are not accessing any campus facilities on the form.

  • To submit your self-certification or claim an exemption in PeopleSoft:
    • Go to HR Center in PeopleSoft
    • Go to Self Service
    • Go to CSU COVID Self-Certification in Personal Information
  • If employees select medical or religious exemption in the self-certification form HR will reach out to collect additional information needed to grant the exemption.
